‘Heart Connect’ is a free information service exclusive to heart failure patients, offered over the phone.
The service compliments the medical care and support you provide to your patients.
Through peer and emotional support, and social reintegration, we empower individuals to regain confidence, independence, and continue to live well with their condition.
Referring a patient is simple.
Just complete a patient referral form and we’ll take it from there.
