Heart and Thrombosis Support and Information for you

Living with a heart or cardiac condition, getting a new diagnosis or recovering from a sudden heart-related illness can be stressful and overwhelming. We’re here to help.

Heart Connect

Heart Connect is our support service for anyone affected by heart or blood clot conditions. This service is delivered by phone. One of our Irish Heart Foundation nurses will call you for a chat to understand what you are going through and how we can best support you. After that first phone call, you can receive up to 5 follow-up phone calls that are tailored to your needs.

Through Heart Connect  you can also access our emotional and social support programmes. These include:

Counselling

You can avail of up to 6 free professional counselling sessions. These sessions can help you cope with your diagnosis, manage stress and lead a full life with your condition.

Peer support groups

Our monthly peer support group meetings are open to anyone with a heart and/or blood clot condition. These groups are an easy and supportive way to meet other people who understand what you might be going through.

Information Only

Some people like to get information before deciding which services feel right for them.

With our Information Only service, we will send you an email with clear, reliable information and guidance tailored to your condition. The email will also include links to relevant pages on our website that have information about your condition, so you can explore more whenever you feel ready.

This option is less hands-on than our Heart Connect service, but it still gives you the information you need to make informed decisions about your health and wellbeing. We will also include details on how to access Heart Connect and our other support services if you would like to take part in the future.

Heart Support Network (Facebook)

Our Heart Support Network is for people living with heart conditions and their carers.

With more than 3,000 members, our Heart Support Network Facebook group provides advice, information and support from a strong community of people affected by heart disease and allows you to share your own thoughts and concerns. Group activities include free online exercise classes, information talks, health advice and peer-to-peer support. Sign up now.

Nurse Support Line

Call our Nurse Support Line if you have a question, a worry or just feel like talking about your condition to someone who understands, our nurses are here for you.  Our Nurse Support Line is available on 01 668 5001, from 9am to 5pm every Monday, and from 9am to 1pm, Tuesday to Friday or email support@irishheart.ie.

You can call our Nurse Support Line as often as you need – no question is too big or too small. Our nurses can also guide you through any the services available and help you with the next steps if you feel you need more support.
