Heart Connect is a dedicated support service for people living with heart failure in the community.
It is currently a pilot project running in South Tipperary, Carlow/Kilkenny, Waterford and Wexford.
Learn moreThis service is free and open to anyone living with a clinical diagnosis of Heart failure anywhere in Ireland. It supports you to manage your condition and meet other people who are living with heart failure like you.
What does this service entail?
Once we receive a referral from your Nurse, GP or clinic, with your consent, you will get a phone call from one of our patient support team to see how you are getting on, to update you on our services, and ensure you have the support you need.

Our Heart Support Network Facebook group is a strong community of people living with heart disease who offer each other advice, information and support. Group activities include free online exercise classes, information talks, health advice and peer-to-peer support. Sign up now.
