As you move from hospital to home the Irish Heart Foundation is here to support you. We know this can be an overwhelming and lonely time so we have worked with many health professionals including cardiac nurses to give you information, advice, and support on living with your heart failure.
Our Heart Failure Services are dedicated to addressing the unmet psycho-social needs of individuals living with heart failure in Ireland. Through these services, we aim to improve the quality of life, reduce feelings of isolation, provide timely and accurate information, and raise awareness about effective symptom management.
The Irish Heart Foundation’s Heart Connect Service is an information service offered over the phone in addition to the usual medical care exclusively for Heart Failure Patients. It offers support around coping with the psychological impacts of the condition, and reinforces education around symptom-management, encouraging the use of social supports and signposting services.
Join one of our monthly peer-led heart support meet ups. These monthly meetings are open to people with all heart conditions. It is an opportunity to meet others who are on a similar journey.
